Whether you are looking for support for yourself, someone close to you, or someone you work with, this is the place to begin.
Who can contact us
- Individuals
- If you are looking for support for yourself, you can contact us directly. You do not need a referral.
- Families and friends
- You are welcome to enquire on someone’s behalf. We will want to involve them in any decisions from the start.
- Social workers
- Send us the basics here and we will arrange to receive the full referral and assessment through the appropriate secure route.
- Commissioners
- We are happy to discuss individual packages, transitions and supported living developments.
- Health and care professionals
- Hospital discharge teams, GPs, nurses and therapists can refer with the person’s consent.
